Information regarding medical history, symptoms, and Long-Term Care (LTC) facility status provided for triage purposes. We also collect personally identifiable information (including name, email, password, communications); payment details (including credit card information), comments, feedback, product reviews, recommendations, and personal profile.

When you conduct a transaction on our website, or fill out a form on our website, submit a signed Consent Form via fax or in-person, interact with our automated clinical agents via SMS or voice, as part of the process, we collect personal information you give us such as your name, address and email address. By opting into SMS communications, you acknowledge the risk that clinical notifications may be visible on a mobile device and agree to communicate via this channel for the sake of care coordination. How do we use cookies and other tracking tools?
A cookie is a piece of data stored on the user’s hard drive that contains information about the user. By using a cookie on our site, a user does not have to log into the site using a password more than once.
